I acknowledge and agree to follow the Lyndwood Tennis Club Guidelines and Protocols, including:

• Follow all club and facility rules and posted guidelines.

• Club access is limited to registered members and authorized individuals.

• Stay home when feeling unwell and follow applicable health recommendations.

• Practice safe tennis habits and respect other members.

• Follow all instructions from Club management and staff.

I recognize that participating in recreational tennis activities presents inherent risks to my health and safety. In consideration of Lyndwood Tennis Club accepting my membership application and/or program registration, I agree to accept these risks and understand that I am responsible for managing the health and safety of myself and any minors, dependent adults, or guests in my care.

I release and forever discharge Lyndwood Tennis Club, its Executives, Directors, successors, agents, and employees from any claims arising from injury or damage while using club facilities.

I agree to indemnify the Club against claims arising from my actions.

Club management reserves the right to deny access for unsafe or inappropriate behaviour.
